Diabetes care and support program launches at Heights clinic
In alignment with the Texas mission to improve diabetes care and outcomes for patients living with diabetes, Avenue 360 Health and Wellness is excited to offer a new clinical service at its Heights location. The focus of this clinic is to help patients with diabetes learn more about their disease and support them to achieve their goals. Patients will meet with a clinical pharmacist to create an individualized plan of care, improve their knowledge of their condition, and develop tools to be successful in managing their diabetes.
According to the Centers for Disease Control and Prevention, there are more that 37 million Americans that have diabetes and 96 million have prediabetes. Prediabetes is a condition described as having slightly increased blood sugar or glucose levels, suggesting that a person is at risk of developing type 2 diabetes. Unfortunately, about 80% of people with prediabetes are not aware of their condition. Texas has one of the highest rates of adults with type 2 diabetes at 12% of the total population.
Diabetes is a chronic health condition that affects how your body turns food into energy. There are two common types of diabetes. Type 1 diabetes is usually caused by an autoimmune response that causes the body to destroy the pancreas cells that make insulin naturally. Type 2 diabetes is the most common type of diabetes, and it can be caused by a number of different reasons. The most common cause of type 2 diabetes is when the pancreas is not able to produce enough insulin or the body becomes less sensitive to the insulin that is made. A majority of people diagnosed with type 2 diabetes may have other chronic conditions that make their diabetes more difficult to control. Symptoms of uncontrolled diabetes can include tiredness, frequent urination, blurry vision, unexplained weight loss, and thirst.
