MEDICAID RENEWALS
Medicaid renewals have begun. For those Avenue 360 Health + Wellness patients on Medicaid, it’s time to update your information to keep your access to doctor visits, medications, and more.
Your local Medicaid office will send you a letter or a renewal form to complete. Please complete your renewal by the due date printed on the form. If you don’t, you could lose your coverage. Don’t risk losing your coverage due to paperwork.
We understand that redetermining your eligibility for Medicaid can be confusing and we’re here to help. Don’t wait, call 713-426-0027 now. Your coverage depends on it.
