COMPASSIONATE HOSPICE CARE

Avenue 360 Health + Wellness provides comforting, compassionate end-of-life care at our Omega House hospice for Houstonians with HIV/AIDS, serving many from the LGBTQ+ community.

Omega House is an 8-bed hospice located in the heart of the Montrose area, where around-the-clock personalized care is provided through our patient-centered approach, which focuses on preserving dignity and quality of life, with people receiving the comfort, care, treatment, and support they need and deserve.

We rely on donations from the community to provide hospice services at Omega House, free of charge to patients.

TEMPORARY ASSISTED LIVING PROGRAM

We are excited to announce a new program at Omega House, the Temporary Assisted Living Program!
This program provides short-term assisted living services to eligible Persons Living with HIV (PLWH) in the Houston EMA. The program supports clients who require continued care and supervision following a hospital stay but lack a caregiver or stable support system at home.
