Substance Use Recovery Support Services

Avenue 360 Health + Wellness is the only federally qualified community health center (FQHC) in Houston, to offer recovery support services led by licensed clinicians who provide counseling in individual and group settings, to most appropriately meet the needs of clients dealing with diverse stressors and conditions.

Samhsa

Did you know that Avenue 360 Health + Wellness is the only FQHC in the state of Texas, to receive funding to support SBIRT screening from the Substance Abuse and Mental Health Services Administration (SAMHSA).

Why does that matter? Since 2003, SAMHSA has only awarded grant funding to 32 states, territories and tribal organizations to enhance services for persons with, or at risk for, substance use disorders. Our program is among a select few, recognized for effectively introducing a variety of new services that extend the continuum of care for substance-involved individuals, ranging from early intervention with non-dependent substance users to referral of more serious cases to specialized substance abuse treatment.

 

Screening, Brief Intervention and Referral to Treatment

Our SBIRT initiative is an evidence-based modality to screen all patients as part of their routine medical visits, to identify and assist those patients who are experiencing harmful substance use. Since launching in May 2022, we continue to expand, screening more than 7,890 primary care and dental patients to date.

Peer Recovery Coaching

Our patient, clients, and participants have access to support by a Recovery Coach who has lived experience in the recovery from substance use. The Recovery Coach helps peers to create and achieve their own Recovery Wellness Plan for improved health and wellness as they strive to reach their full potential.

Outpatient Substance Use Disorder Counseling and Treatment

Designed to help you recover from addiction to drugs and alcohol, we take a patient-focused approach to our substance use treatment services, available within our health clinics throughout greater Houston.

Priorities for our Outpatient Treatment include: 

  • Create a safe and supportive environment to promote healing each patient’s body, mind, and soul.
  • Use evidence-based and innovative treatment models.
  • Provide affordable and effective care with multiple payment options, including private insurance, Medicaid, and sliding scale self-pay fee.
  • Empower patients to feel confident and secure after treatment.
  • Address co-occurring mental health conditions to improve patient wellness.

Medication Assisted Treatment for Opiate and Alcohol Use Disorders

At Avenue 360 Health + Wellness, we use FDA-approved medications in combination with therapy treatment and to promote sustained recovery. In combination with interdisciplinary interventions, including evidence-based therapies and cognitive behavioral therapy (CBT), we assess each patient for mental health and other healthcare needs that may come up in the assessment process, such as motivational enhancement therapy.

Medication-asssisted treatment (MAT) uses FDA-approved medications in combination with following:

  • Substance Use Screening, Brief Intervention and referral to Treatment (SBRIT)
  • Peer Coaching
  • Outpatient Substance Use Disorder Counseling and Treatment
  • Psychiatry for Medication Assisted Treatment for Opiate and Alcohol Use Disorders
  • Brief Treatment
  • Nicotine Replacement Therapy
  • HARM Reduction
