Mark Levine departure letter

Dear Patients,

This letter is to inform you, my last day with Avenue 360 Health & Wellness will be March 31, 2023. Avenue 360 Health & Wellness will continue to provide Family Medicine/ HIV care and other services. Patients may continue to remain at Avenue 360 for their medical Care, or they may choose to establish care with another physician/provider.

For patients: medical records are confidential; however, a copy can either be transferred toanother physician/provider, with a signed written authorization form from you, or released to you with proper identification. If you choose to see a different physician/clinician, please request an authorization form and return it to our clinic as soon as possible so we may transfer your records to your new physician/provider. Until then, your records will remain on file at Avenue 360 Health & Wellness.
