Dr. Patricia Hayes joins Avenue 360 Health and Wellness as Executive Vice President of Medical/Chief Medical Officer. She is a double board-certified physician specialized in Internal Medicine and Pediatrics. She has successfully created chronic disease management programs for diabetes and high blood pressure and has led multidisciplinary teams that design and implement projects on population health, value-based care models, disease prevention, and health promotion. She has extensive experience practicing in underserved and resource-limited environments.

Dr. Hayes earned her Doctorate of Medicine degree from the University of Texas Health Science Center – Houston and completed her residency in Internal Medicine and Pediatrics at Baylor College of Medicine. She has previously served as Medical Director for Partners in Primary Care/Centerwell helping to oversee clinical operations across 12 clinical sites in the Houston region. She also served as Medical Director for the Department of Family Practice at Legacy Community Health, overseeing clinical care of all family practice sites throughout Houston, Baytown, and Beaumont spanning five clinical specialties including, internal medicine, pediatrics, infectious disease, geriatrics, and endocrinology. Dr. Hayes speaks Spanish, Portuguese, and English.
