Dear Patients,
This letter is to inform you, my last day with Avenue 360 Health & Wellness will be 7/14/2023.
Avenue 360 Health & Wellness will continue to provide primary care and other services. Patients may
continue to remain at Avenue 360 for their medical Care, or they may choose to establish care with
another physician/provider. I hope you will continue to utilize the services here at Avenue 360.

For patients: medical records are confidential; however, a copy can either be transferred
to another physician/provider, with a signed written authorization form from you, or
released to you with proper identification. If you choose to see a different
physician/clinician, please request an authorization form, and return it our clinic as soon
as possible, so we may transfer your records to your new physician/provider. Until then,
your records will remain on file at Avenue 360 Health & Wellness.

We both appreciate the opportunity to assist in your care. If there is any further
