At Avenue 360 Health + Wellness, we believe income or insurance status should not determine the quality of care you receive.

As a community health center, we provide high-quality and caring services to promote healthy people and communities throughout Houston. We use a sliding fee scale to determine the cost of your care, based on your income and household size. No one is turned away for their inability to pay.

Currently Uninsured?

Navigating healthcare and coverage options can be overwhelming, confusing, and even intimidating. At Avenue 360 Health + Wellness we are here to help. Our Eligibility specialists can help guide you through a combination of grants, government assistance, and sliding fee scale options to get you covered. Whether you’re uninsured or simply seeking affordable options, they’ll work tirelessly to find the affordable options just for you. Sliding Fee Scale

Even if you don’t qualify for assistance programs, there’s still hope. Our Sliding Fee Scale takes into account your income and family size. Meet with one of our Eligibility specialists to assess your situation. They will help determine the best rate based on your unique circumstances. No one is turned away—we serve patients regardless of their ability to pay and provide discounts for essential services. Accepted Insurance

We accept the following insurance: Aetna, Ambetter, Amerigroup, Blue Cross Blue Shield, Cigna, Community Health Choice, Medicaid, Medicare, Texas Children’s Health Plan, United Healthcare, and many others.

For prescriptions, Avenue 360 Pharmacy provides lower cost medications to those without insurance.

Please Note: Effective, Tuesday April 30, 2024, we will NO LONGER accept MyBlue Health Plan coverage.

Medicaid Renewals Have Begun

For our patients on Medicaid, it’s time to update your information to keep your access to doctor visits, medications, and more.

Your local Medicaid office will send you a letter or a renewal form to complete. Please complete your renewal by the due date printed on the form. If you don’t, you could lose your coverage. Don’t risk losing your coverage due to paperwork. We understand that redetermining your eligibility for Medicaid can be confusing and we’re here to help.
