The focus of this clinic is to help patients with diabetes learn more about their disease and support them to achieve their goals. Patients will meet with a clinical pharmacist to create an individualized plan of care, improve their knowledge of their condition, and develop tools to be successful in managing their diabetes.<\/span><\/p>\n<p class=\"s5\"><span class=\"s2\">According to the\u00a0<\/span><a href=\"https:\/\/www.cdc.gov\/diabetes\/data\/statistics-report\/index.html\"><span class=\"s3\">Centers for Disease Control and Prevention<\/span><\/a><span class=\"s2\">, <\/span><span class=\"s2\">there are more that 37 million Americans that have diabetes and 96 million have prediabetes. <\/span><span class=\"s2\">Prediabetes is a condition described as having slightly increased blood sugar or glucose levels, suggesting that a person is at risk of developing type 2 diabetes. Unfortunately, about 80% of people with prediabetes are not aware of their condition. <\/span><a href=\"https:\/\/www.cdc.gov\/diabetes\/library\/reports\/reportcard\/national-state-diabetes-trends.html\"><span class=\"s4\">Texas<\/span><\/a><span class=\"s2\"> has one of the highest rates of adults with type 2 diabetes at 12% of the total population. <\/span><\/p>\n<p><span class=\"s2\">Diabetes is a chronic health condition that affects how your body turns food into energy. There are two <\/span><span class=\"s2\">common <\/span><span class=\"s2\">types of diabetes. Type 1 diabetes is<\/span><span class=\"s2\"> usually <\/span><span class=\"s2\">caused by an autoimmune response<\/span><span class=\"s2\"> that causes the body to destroy the pancreas cells that make insulin naturally. Type 2 diabetes is the most common type of diabetes, and it can be caused by <\/span><span class=\"s2\">a number of<\/span><span class=\"s2\"> different reasons.
