SOAR Case Management

Our case management program offers support to homeless adults with mental illness, physical disability and/or substance abuse issues for the purpose of expediting the SSDI/SSI and medical benefits application process. Avenue 360 employs 2 SOAR case managers who serve as the appointed representative before the Social Security Administration for clients who are homeless and disabled, using the SOAR process for filing disability claims.

Health Education & Street Outreach

Avenue 360’s approach to health extends beyond the clinic’s four walls. Our dedicated team works on empowering and enriching communities throughout the Houston area to improve health outcomes, address social determinants of health, and establish partnerships that focus on education, prevention, and health & wellness to build healthier and stronger communities. The Street Outreach team provides much-needed services to individuals and families living in places not meant for human habitation (streets, freeway underpasses, cars, encampments, etc.) in Houston/Harris County. Our team is embedded in the community by participating in food distribution events, attending cooperative community meetings, planning community baby showers, and being a part of community and city-led task forces. We work to identify and establish collaborations with community organizations and partners, develop and host education sessions in the community on topics such as healthy eating habits, women’s health, sexual health, and cardiovascular health, and provide free health screenings such as blood pressure, glucose, HIV, Hepatitis C, and Syphilis testing.

Community Partnerships

From our virtual town halls with Univision to a presence at events like Houston Pride and job training sites, we build relationships with community members. Avenue 360 and its community partners do not simply focus on health care, but also on individual behavior, exposure to health threats, and social and economic conditions. This level of community integration helps build rapport with community members and potential patients, but also allows for Avenue 360 to meet with established partners and supporters in a non-traditional setting that identifies, evaluates, and prioritizes gaps, which allow us to develop strategies to respond to our communities unmet needs. Partnerships are established with apartment complexes, universities and colleges such as The University of Houston, Texas Southern University, Houston Community College and Lonestar College, school nurses, WIC offices, YMCAs, places of worship, shelters, community-based organizations, and local precinct offices.
