In addition to ensuring quality, affordable health care is available to lower-income populations in the Houston region, we are committed to the wellbeing of entire communities. Does a community in Southwest Houston, for instance, have enough healthy food and clean air? What about sufficient housing? Is one community experiencing discrimination or racism as a daily part of life? These environmental/social factors negatively impact individual and community health.

Our vision is to close the health disparities certain communities face in the region. We continue our longtime focus on housing, whether apartments for the homeless, hospice care, or adult day activities.

Part of community wellness to us means providing basic support and health education.

Permanent Supportive Housing

Permanent Supportive Housing (PSH) programs are designed to promote residential stability and positive health outcomes by offering indefinite stay housing options to individuals and families who are experiencing homelessness and where at least one member of the household has a documented disability. Services include rent subsidies, case management, transportation assistance, and advocacy.

To determine if you are eligible for Permanent Supportive Housing you must be assessed at one of Houston’s designated Coordinated Access sites. Information can be obtained at Houston’s Coalition for the Homeless ​www.homelesshouston.org.
