This letter is to inform you my last day with Avenue 360 Health & Wellness will be January
31, 2025. Avenue 360 Health & Wellness will continue to provide care and other services. Patients
may continue to remain at Avenue 360 for their medical Care, or they may choose bestablish care with
another physician/provider. I hope you will continue to utilize the services here at Avenue 360.

For patients: medical records are confidential; however, a copy can either be transferred
toanother physician/provider, with a signed written authorization form from you, or
released to you with proper identification. If you choose to see a different
physician/clinician, pleaserequest an authorization form, and return it our clinic as soon
as possible, so we maytransfer your records to your new physician/provider. Until then,
your records will remain on file at Avenue 360 Health & Wellness.

We both appreciate the opportunity to assist in your care. If there is any further
