Avenue 360 recognized for championing heart health and diabetes care
At Avenue 360 Health + Wellness, we work hard to improve our community’s health, one person at a time. We’re excited to announce that the American Heart Association (AHA) and American Medical Association (AMA) have honored us with two awards. We received the Target: BP Achievement Award for our efforts to control blood pressure and recognition for our work in Target: Type 2 Diabetes. These awards show how we turn science-based guidelines into real care that changes lives.
Through #TargetBP, we’re helping improve the nation’s blood pressure control. We follow the latest guidelines to ensure better health for patients with high blood pressure. Our work in Target: Type 2 Diabetes also shows our dedication to quality care for people with diabetes. These programs are more than just initiatives—they’re our commitment to using proven knowledge to make a difference every day.
